March 2007 - Continuity of care in type 2 diabetes: patients', professionals' and carers' experiences and health outcomes
A research summary that outlines an investigation of the relevance of continuity in the care of type 2 diabetes. The researchers set out to evaluate and measure patients', carers' and providers' experiences of continuity of care in type 2 diabetes and to determine whether continuity of care was associated with improved clinical and patient outcomes (Gulliford et al, 2006).
